Hackensack Meridian Health and its affiliates (HMH) MyChart (“MyChart”) is an Internet application provided as a service to its users that (i) enables patients to have secure web-based access to information from their medical record maintained by Hackensack Meridian Health and its health care providers (collectively referred to as HMH) in electronic format, (ii) provides patients and their guarantors the ability to pay for services; and (iii) provides a secure mechanism to communicate with providers. HMH is offering this service to patients, so patients have a convenient means of reviewing information from their medical record, communicating with their HMH health care providers, arranging for clinical services/appointments, and paying for services rendered.
The electronic and accessible nature of medical record information maintained on MyChart presents unique security and privacy issues as addressed below. Therefore, certain necessary measures for protecting the security and privacy of such information are the responsibility of the user as detailed below. By requesting a MyChart account and continuing to use MyChart, you are agreeing to these Terms of Use, which include legal and privacy waivers, disclaimers, and personal responsibility requirements.
The information from your medical record available through MyChart may not constitute your entire medical record. The information available through MyChart is limited to that information that is maintained within HMH’s primary electronic medical record system (Epic). You may have records stored in other systems that cannot be viewed through MyChart. There also might be technical or legal limitations on what is viewable to you in MyChart. You will continue to have access to your full medical record by contacting the location of your health care providers directly or can also request a copy of your full medical record maintained by HMH by submitting a request through your MyChart account.
You understand that some of the healthcare providers at HMH are not employees or agents of HMH. These healthcare providers are independent practitioners or contractors who have been granted certain privileges to use HMH’s facilities for the care and treatment of their patients. These independent healthcare providers include, but are not limited to, Emergency Department physicians, anesthesiologists, cardiologists, neonatologists, obstetricians, pathologists, radiologists, surgeons, the on-call physician and other consultants who may treat you.
YOU ACKNOWLEDGE THAT YOU ASSUME FULL RESPONSIBILITY FOR YOUR USE OF HMH MYCHART. HMH SHALL NOT BE LIABLE FOR ANY PERSONAL INJURY, INCLUDING DEATH, ARISING FROM YOUR USE OR MISUSE OF MYCHART OR ANY INFORMATION OR CONTENT THEREIN. Remedies under these Terms and Conditions of Use are sole and exclusive and are limited to those expressly provided for in these Terms and Conditions of Use.
Guarantors enrolling in MyChart on or after 7/1/2025 will be automatically enrolled in electronic statements. To opt out of electronic statements access the Menu Options/Billing Summary/Paperless Preferences
You are not required to utilize MyChart and may discontinue usage at any time. You acknowledge that you are expected to use MyChart appropriately. Overusing the messaging feature of MyChart or using inappropriate or threatening language are a violation of these Terms and Conditions. You acknowledge that MyChart is being provided to you without charge. Therefore, HMH reserves the right to terminate your access to MyChart at any time, with or without cause. Should HMH determine that you have violated these Terms of Use, as may be modified from time to time, HMH may discontinue your usage of MyChart immediately, without providing prior notice. In addition, MyChart may not always be available due to system maintenance, outages, or other issues beyond the control of HMH.
Without intending to limit HMH’s sole discretion to provide access or to limit or terminate access to MyChart in any way as described above, you agree that you will not:
MyChart enables you to send requests, questions and other communications, regarding your medical record information, electronically to your health care providers. Such communications will be routed to an appropriate individual within your health care provider's department and/or practice. Therefore, individuals other than your personal health care provider may review your medical record information as required to address your request. If your communication contains sensitive information that you want shared only with your personal health care provider, you should contact such provider directly and not through MyChart. All communications sent through MyChart will become part of your permanent medical record and may be used and disclosed as permitted by law.
Communications via MyChart for urgent matters is not appropriate. In those circumstances, contact your personal health care provider directly. In emergency situations, please dial 911.
Communications from your health care providers via MyChart will contain important information regarding your health care. It also is possible your health care providers may decide not to communicate such information by other means if communicated through MyChart. Therefore, it is important for you to regularly monitor your message inbox in MyChart and your personal e-mail for message notifications. You agree that you will not hold HMH liable for loss, injury or claims resulting from your failure to read messages in a timely manner.
You will be notified via e-mail when there is new medical information to be viewed on MyChart. Notifications may include, but are not limited to, provider messages, test results, prescription renewals, letters, questionnaires, after visit summaries and appointment status and reminders. Any person with access to your e-mail will be able to see this notification. This could include your spouse, employer or anyone else that can access your e-mail account. Although no private medical information will be sent, the notification that new medical information is available by accessing MyChart may be information that you would not want others to know. You should take this into account when providing an e-mail address and when sharing any information required to access your personal e-mail account. By accepting these Terms of Use and continuing to use this site, you agree to receive communications by e-mail.
You will also be given the opportunity to access patient educational materials prepared by third parties through MyChart. These educational materials are provided for your convenience only. HMH does not assume any liability or responsibility for the content in these materials. Any information in these materials should be discussed with your healthcare provider before making any treatment decisions based on that information.
MyChart Bedside is one application in MyChart that may be offered to you for use to access clinical information while you are admitted as an inpatient to a HMH facility. The Terms of Use in this agreement will also apply to your use of MyChart Bedside. Any reference to MyChart in the following Terms of Use will include MyChart Bedside. HMH is under no obligation to offer any patient the ability to use MyChart Bedside and has sole discretion in offering such use. Use of MyChart Bedside is free of charge to you as the patient via your personal device. Some of the information in MyChart Bedside may only be available for a limited time after you are discharged, and not all past inpatient encounters will be available for viewing. If you do not have a MyChart account yet, you will be given the opportunity to create one during your hospitalization.
The information in MyChart Bedside may not fully reflect clinical data, such as the clinical staff assigned to your care team. If you have any questions, you should ask a clinical team member. MyChart Bedside should not be used as a replacement for necessary communication with your clinical care team. Any question you may have about your medical care or lab results should be directed to your care team. You should not rely on information provided by MyChart Bedside to make decisions.
You may be offered the use of a Hackensack Meridian Health-owned tablet to use while an inpatient to access MyChart Bedside. You will be able to access and use several applications (“apps”) on the tablet using your own personal account log-in. You will not be able to download any apps that have not been preloaded onto the tablet. By agreeing to use MyChart Bedside, you understand that the tablet provided to you is a loan. You agree not to take the tablet outside of your hospital unit and agree to take reasonable measures to protect the tablet from loss or damage. You may be responsible for any loss or damage to the tablet caused by you. At the end of your hospitalization, you agree to return the tablet to the docking station within your hospital room or to a member of your clinical team. All your personal health information and any other data, including any log-in information, that was stored in the tablet will be erased once you are discharged from the hospital.
If you do not have a MyChart account yet, you will be given the opportunity to create one during your hospitalization.
MyChart Bedside has a feature called “Happening Soon”. If this feature is available to you, you may enter personal events, but those will not be visible to the clinical staff involved in your care. Please note that entering this information is for your purposes only.
Hackensack Meridian Health reserves the right to remove the tablet and/or discontinue your MyChart Bedside account or a portion thereof for any reason, including, but not limited to, abuse or mishandling of the application or tablet, interference with clinical care, behavioral issues, or failure to comply with these Terms of Use or Hackensack Meridian Health policy. Such termination may occur without any notice to you. These actions are in addition to and not in lieu or limitation of any other right or remedy we may have available at law. Further, we shall not be liable to you or any third party for any such termination of discontinuation.
Information from your medical record that is accessible through MyChart is intended for your access only (subject to the discussion below under “Proxy Access”). You will have a unique username and password for MyChart, which will provide you with such access, which is crucial to protecting the security of such medical record information. Therefore, you should select a password that is confidential and it should not be shared with anyone. In the event you believe your password has been compromised, you may change it anytime through MyChart.
HMH will utilize certain security measures within the scope of MyChart such as verification of identify through the use of specific activation codes and the tracking of any activity within the account. However, once you access your medical information through MyChart, you are solely responsible for maintaining the security of such information on your end as well as the information required to access such information (i.e., username and password). Specifically, you are responsible for the results or consequences of your communications regarding, or the sharing of, information accessed via MyChart.
An individual must be age 13 or older to request a personal MyChart account with healthcare provider approval. Parents or legal guardians of minor children may request access to the minor child's medical information by following the terms outlined in the "Proxy Access" section of these Terms of Use. However, the full spectrum of MyChart services may not be available under Proxy Access for the records of minor children between the ages of 13 and 17 based upon applicable federal, state, or local laws(inclusive).
A parent or legal guardian is permitted to have access to his or her minor child’s online health information through MyChart. This type of access is known as Proxy Access. At 13, a minor child may apply for their own MYCHART account at their healthcare provider’s discretion. A parent or legal guardian of a minor child 13-17 years of age may apply for Proxy Access to the minor account and will be subject to the same terms and condition of this agreement. Access may be limited due to technical limitations and certain federal, state or local privacy laws. At age 18 or upon an order of emancipation, the minor account and Proxy Access is automatically terminated. The minor will be asked to open a new MYCHART account for adults.
Patients 18 years or older may authorize other individuals to have access to their online health information. The “Sharing My Records” in MyChart can be used to send invitations to give access to a proxy.
Legal representatives of patients who lack the capacity to make their own health care decisions are permitted full proxy access to the patient’s online health information upon presentation of appropriate legal paperwork.
Once a patient is deceased, a proxy’s access to the patient’s MyChart account is automatically terminated. The personal representative of a deceased patient is permitted to request full proxy access to the deceased patient’s online health information upon presentation of appropriate legal paperwork. Access to a decedent’s MyChart information is valid for a maximum of one year and then access is automatically terminated.
If the proxy’s legal relationship with the patient changes, the proxy must inform HMH immediately by contacting the MyChart Help Desk. HMH reserves the right to revoke proxy access at any time for any reason.

HMH DOES NOT WARRANT OR GUARANTEE THE QUALITY, ACCURACY, COMPLETENESS, CORRECTNESS, TIMELINESS OR USEFULLNESS OF ANY INFORMATION PROVIDED THROUGH MYCHART OR THE INTERNET GENERALLY AND EXPRESSLY DISCLAIMS ALL WARRANTIES CONCERNING MYCHART WHETHER EXPRESS OR IMPLIED, STATUTORY OR OTHERWISE, INCLUDING ANY WARRANTIES OF MERCHANTABILITY, TITLE, NON-INFRINGEMENT AND/OR FITNESS FOR ANY PARTICULAR PURPOSE, AND WARRANTIES OF PERFORMANCE, AND ANY WARRANTY THAT MIGHT OTHERWISE ARISE FROM COURSE OF DEALING OR USAGE OF TRADE. Under no circumstances shall HMH be liable for incidental, special, indirect, direct or consequential damages or loss of profits, interruption of business, or related expenses which may arise from your use of or inability to use MyChart and other services provided hereunder, including, but not limited to, those resulting from defects in software, communication lines, virtual private networks, the Internet, or your Internet service provider, access system computer hardware or software or other service or device that is used to access MyChart and/or documentation, or loss or inaccuracy of data of any kind, whether based on warranty, contract, tort, or any other legal theory, and whether or not HMH is advised of the possibility of such damages.
HMH DOES NOT WARRANT OR GUARANTEE THAT USE OF MYCHART WILL BE UNINTERUPTED FREE FROM UNAVAILABILITY OF INFORMATION, DOWNTIME, SERVICE DISRUPTIONS, INTENTIONAL OR INADVERTENT INTRUSIONS, VIRUSES OR WORMS OR OTHER MECHANISMS THAT MAY ATTACK YOUR COMPUTER OR OTHER ELECTRONIC DEVICE. YOU ACKNOWLEDGE THAT YOU ARE SOLELY RESPONSIBLE FOR ENSURING THAT YOUR COMPUTER HAS ADEQUATE PROTECTIONS AGAINST ANY OF THE FOREGOING. IN ADDITION, YOU ACKNOWLEDGE THAT YOUR INFORMATION WILL BE TRANSMITTED OVER THE INTERNET AND THAT THERE IS NO GUARANTEE OF ACCURACY OR SECURITY.
HMH takes no responsibility for, and disclaims liability from, any results or consequences from your intentional or unintentional sharing of, provision of access to, or disclosure of your username, password, activation code or medical record information available through MyChart.
You hereby agree to indemnify, defend and hold harmless HMH from against any damages, losses, liabilities, judgments, costs or expenses (including without limitation reasonable attorneys' fees and costs, including without limitation any incurred in enforcement of this indemnity) arising out of any claim relating to your use of MYCHART, or your breach or violation of these Terms or any other term or condition contained on the MYCHART.
All trademarks, service marks, design, information, text, graphics, images, pages, interfaces, links, software, and other items and materials contained in or displayed on MYCHART and the selection and arrangements thereof, are our property or that of their respective owners. Except as expressly provided in this section, your rights to use any of the foregoing are only in the context and within the confines of the MYCHART or the applicable third party site or service, and in all cases in accordance with these Terms and the terms of those sites. All rights are reserved by the applicable owner.
Any and all trademarks, service marks and logos on the site are owned by us or by their respective owners.
You may download and reprint a single copy of the materials from the Site for your own personal, noncommercial use only, provided you include all applicable notices and disclaimers. Any other use of the materials is strictly prohibited without our prior written permission and the permission of the applicable rights holder(s).
These Terms and the Privacy Policy (and all documents referenced in the Privacy Policy) constitute the entire agreement between you and us with respect to your use of MYCHART, and supersede all prior and contemporaneous communications and documents, whether electronic, oral or written, between you and us with respect to MYCHART.
You agree that if we do not exercise or enforce any legal right or remedy which is contained in these Terms (or which we otherwise have under applicable law), such omission will not be taken as a formal waiver of our rights and shall not be construed to be a modification of these Terms.
If any court of competent jurisdiction rules that any provision of these Terms is invalid, then that provision will be revised, reduced or removed from these Terms, to the minimum extent necessary to render it valid, without affecting the rest of these Terms. The remaining provisions of these Terms will continue to be valid and enforceable.
These Terms and Conditions and all disputes, claims and controversies (“Disputes”) arising out of or related to your access or use of HMH MyChart shall be governed and construed exclusively in accordance with the laws of the State of New Jersey (even if you are located outside the State of New Jersey at the time you agree to them). By agreeing to these Terms and Conditions you irrevocably agree to submit to the exclusive jurisdiction of the state and federal courts located in or serving the State of New Jersey for any Dispute with HMH or our affiliates, subsidiaries, employees, contractors, officers, directors, telecommunication providers, or content providers.
